In accordance with the “No Surprises Act” of 2022, you are entitled to a Good Faith Estimate (GFE), which provides you an estimate of out of network health care costs. A GFE must be provided at least three (3) business days before the requested appointment/ service or no later than three business days after the date of request if the patient requests a GFE without scheduling the service.

The Good Faith Estimate does not include any unknown or unexpected costs of treatment.
